1. Introduction

Welcome to Lev Pediatrics. By accessing our services, website, and communication platforms, including SMS and email, you agree to comply with these Terms and Conditions. If you do not agree, you should not use our services.

2. Consent to Treatment

By receiving medical services at Lev Pediatrics, you consent to necessary medical treatment, including:

• Physical examinations
• Laboratory testing
• Other diagnostic procedures as deemed necessary by your provider

3. Release of Medical Information

You authorize Lev Pediatrics to share medical records with mutually agreed-upon physicians or medical facilities as needed for your care.

4. Self-Pay Practice and Charges

• Lev Pediatrics is a self-pay practice. We do not bill insurance and are not in network with any health plan
• Prices are shown before you pay. Current visit prices, the after-hours fee, and membership pricing are listed on our Pricing page
• Payment for a visit is collected when you book online or at the time of service
• On request, we provide a superbill you may submit to your insurer for possible out-of-network reimbursement. We cannot guarantee that your insurer will reimburse any amount

5. Card Payments and After-Hours Authorizations

• Payments are processed by Stripe, a PCI-DSS Level 1 payment processor. Lev Pediatrics does not store your full card number
• By providing a card, you authorize Lev Pediatrics to charge it for the services you book and, for members, for your monthly membership
• After-hours appointments require provider approval. When you request one, your card is authorized for the visit amount but not charged. If the provider approves the request, your card is charged and the visit is confirmed. If the request is declined, the authorization is released and nothing is charged

6. Family Membership

• The Family Membership is billed monthly at $200 for the first child and $100 for each additional child, up to a family maximum of $500 per month
• Membership includes the services described on our Pricing page at the time you enroll. For members, well visits for children under 2 are $100 and the after-hours fee is waived
• Member pricing applies only to children on your plan. We may verify membership before a visit; if member pricing was applied for a family without an active membership, we may charge the difference from the standard price
• To add or remove a child, contact us and we will update your plan; any adjustment appears on your next invoice
• You may cancel at any time, which stops future charges. The current billing period is non-refundable

7. Payment Policies & Fraud Prevention

• By making payments, you agree to any terms set by your card issuer
• Any attempts to dispute valid charges through false chargebacks may be considered fraud
• Lev Pediatrics may take legal action, including collection efforts, reporting to consumer agencies, or civil litigation for theft of services
• False disputes may also result in the loss of card payment privileges at our practice

8. Mobile Messaging & Communication Policy

Lev Pediatrics may contact you via SMS and email for appointment reminders, billing notifications, and important healthcare updates. By providing your mobile number and email, you consent to receive these communications.

Message Frequency

You may receive messages weekly or as needed, depending on your medical care and account status.

Potential Costs

• Standard message and data rates may apply as determined by your carrier
• We are not responsible for any costs incurred by your mobile carrier related to our communications

Opt-In Consent

By providing your mobile number and email, you explicitly consent to receive SMS and email communications from us. This includes notifications related to:

• Appointments and scheduling
• Billing and account updates
• Medical follow-ups
• Other healthcare-related information

Opt-Out Instructions

• You may opt out of SMS communications at any time by replying STOP
• After replying STOP, you will receive a final confirmation message and will be removed from all messaging systems within 24 hours
• To opt out of email communications, use the "unsubscribe" link included in our emails
• Opting out of communications may affect your ability to receive appointment reminders and other important updates

Privacy & Data Sharing

• We do not share your mobile or email information with third parties for marketing purposes without your explicit consent
• Your data is used solely for healthcare-related communications and necessary operational purposes

9. Help & Assistance

If you need assistance regarding SMS or email communications, text HELP for support. You will receive a response with our phone number, email address, and/or website so we can assist you.

10. Updates to Terms & Conditions

Lev Pediatrics reserves the right to update these terms at any time. Continued use of our services constitutes acceptance of the revised terms.
