As a patient of Lev Pediatrics, you (and, where you are a minor, your parent or legal guardian acting on your behalf) have the rights and responsibilities described below, consistent with the Florida Patient's Bill of Rights and Responsibilities (Fla. Stat. § 381.026).
You Have the Right To:
- Be treated with courtesy and respect, with appreciation of your individual dignity, and with protection of your need for privacy.
- A prompt and reasonable response to your questions and requests.
- Know who is providing medical services and who is responsible for your child's care.
- Change providers if other qualified providers are available.
- Know what patient support services are available, including whether an interpreter is available if you do not speak English.
- Bring a person of your choosing with you to the patient-accessible areas of the office while your child receives care, unless doing so would risk the safety or health of the patient, other patients, or staff, or cannot reasonably be accommodated.
- Know what rules and regulations apply to your conduct.
- Be given information by your provider concerning diagnosis, planned course of treatment, alternatives, risks, and prognosis, in terms you can understand.
- Refuse any treatment, except as otherwise provided by law, and to be informed of the medical consequences of doing so.
- Be given, upon request, full information and necessary counseling on the availability of known financial resources for care.
- Receive, upon request and prior to treatment, a reasonable estimate of charges for medical care.
- A copy of a reasonably clear and understandable, itemized bill and, upon request, an explanation of charges.
- Impartial access to medical treatment regardless of race, national origin, religion, disability, or source of payment.
- Treatment for any emergency medical condition that will deteriorate from failure to provide treatment.
- Know if medical treatment is for purposes of experimental research and to give consent or refuse to participate.
- Express grievances regarding any violation of these rights through the process described below.
You Are Responsible For:
- Providing accurate and complete information about your child's present complaints, past illnesses, hospitalizations, medications, and other health matters, to the best of your knowledge.
- Reporting unexpected changes in your child's condition to the provider.
- Reporting whether you understand the contemplated course of action and what is expected of you.
- Following the treatment plan recommended by the provider.
- Keeping appointments and arriving on time, or notifying the office if you will be late or unable to attend.
- The consequences of refusing treatment or not following the provider's instructions.
- Ensuring that the financial obligations for your child's care are met promptly.
- Following the office's conduct rules and regulations, including expectations for respectful behavior toward staff and other patients.
